By participating in activities with OneBlood, Inc., I hereby grant permission for the use of my name, photographs, videos, audio recordings, or any other visual/audio representations involving me or my child. If the participant is under 18 years old, a parent or legal guardian must fill out this form and agree to the terms of this consent form on behalf of the person identified above.

This grants OneBlood, Inc. the right to reproduce, publish, or distribute these materials without limitation. I acknowledge that I, or the participant, will not receive any compensation for the use of these materials. I release OneBlood, Inc., its representatives, and associated organizations from any liability related to the use or publication of these materials.

Additionally, I waive any rights to privacy or publicity regarding these materials and their derivative works.

This release absolves OneBlood, Inc. and its affiliates from any future claims based on the use of my name, likeness, or voice in any capacity related to their marketing programs.
